| About this Form |
Wherever possible, complaints are best resolved by front-line staff. Where a concern or complaint cannot be resolved this way, a formal complaint may be lodged via this form.
Complaints will be handled as per the Department of Planning and Environment’s Complaints Handling Policy which defines a complaint as:
"An expression of dissatisfaction made to or about an organisation, related to its products, services, staff, or the handling of a complaint, where a response or resolution is explicitly or implicitly expected or legally required."

Contact details are collected for the purposes of dealing with the complaint only and, if submitted, they will be stored in accordance with the Privacy and Personal Information Protection Act 1998 (PPIPA).

You may submit a complaint without providing contact details, but anonymous complaints will only be investigated where sufficient detail is provided to substantiate a valid complaint.
Any investigation may be hampered by our inability to seek further information and details from you. Also, it is not possible to update a complainant on outcomes when no contact details have been provided.
